Consent to Provide Information

Submit the below consent form to grant your partner access to view your entire installed base. This will allow your partner to help drive your business outcomes, simplify your renewals, plan for end-of-life, improve security and more.
Cisco is authorised to release installed base and/or product usage telemetry information described in Section 2, for the company indicated in Section 1 below, to the authorised parties identified in Section 3.

1. Consent to Provide Information

  • Cisco software and/or appliance connection dates
  • Cisco software version level
  • Quantity and type of devices leveraging Cisco software (per Customer site)
  • Percentage of devices utilizing Cisco software
  • Cisco CX Lifecycle stage
* The Installed Base and Product Usage data is not limited to what was purchased from the incumbent partner
The following types of information will not be shared: Personal Data; Other Incumbent Partner Name(s); Discounts; Service Levels; Sales Orders; Service Contract Numbers; and Purchase Order Information.

3. Authorised Parties
The person(s) and company will be allowed to receive the information mentioned in Section 2.

4. Validity Time Period
Your consent will be valid until you revoke.

5. How to Revoke Consent
Customer can revoke this consent at any time, by emailing customer-consent@cisco.com.

6. Disclaimer
By clicking 'Submit' below, Customer consents to and requests that Cisco Systems, Inc. or its affiliates periodically provide or grant access to the information specified above to the Partner and affiliated entities. Customer agrees that Cisco will not be liable for Partner's use of the data. Your contact information will be added to Cisco's database and may be used for future communications.
